1 CBC with differential and platelet count; AST, total bilirubin, creatinine, urinalysis, creatinine clearance, magnesium, calcium, phosphorus, BUN, electrolytes, alkaline phosphatase, albumin, bicarbonate-follow-up is required more often and for a longer period if abnormalities are detected.

2 Follicle-stimulating hormone, luteinizing hormone, estradiol/or testosterone (when > 15 years old).

3 Baseline at entry into long-term follow-up, then as per United States Preventive Task Force Recommendations.

4 Baseline at age 18, and repeated as clinically indicated.

5 Periodic evaluation is required in cases of detected abnormalities, or per recommendation of a cardiologist. The COG recommends EKG and Echocardiogram every two years for patients with this amount of anthracycline exposure.

6 Then regularly per recommendations of your orthopedic surgeon.
